Terrain. Mountains, valleys, lakes, rivers etc. A test for the skills of the civil engineers.
Supply logistics which get more difficult the deeper you penetrate into territory.
Personnel, keeping large numbers of labourers fed, sheltered, etc.
Hostility and possible sabotage from people whose land the railroad passes through.
Unforeseen problems like floods, tornados and other extreme weather.
Safety problems, storage and use of dynamite needed for blasting cuttings, danger of rock falls, etc.
Costing. Always a danger that big projects are going to run over budget.

Well the Central Pacific Railroad was in the Sierras they had a hard in with cutting and blasting through the mountain rock and they were bogged down by snow, and they had to get rails, locomotives, and other rolling stock from the east. On the Union Pacific that was no problem, but when they got to the plains, it was hard to get wood for ties, and they were losing men in the towns, also indian raids happened without warning and they had to have help from the army. Hope this all helps.

Biggest problem: The Sierra Nevada Mountains. In particular getting through Donner and Emigrant Passes and through the Truckee River canyon. Compared to that, the rest was a piece of cake.
If you found five other problems equal to that, my hat is off to you.
